For me it varies. Mostly a slow increase but it's been known to as you say ambush me.

I have both. First thing in the morning I'm stiff and have neck and back pain, then it gets better. I've also had pain that hits hard and made me have to go to the hospital for. This is such a crazy disease.

Boy, do I hear you. Nightly jumping and jerking is like being poked with a cattle prod. Very painful and relentless. I feel bruised and battered after a bad session.
What I found helps settle is a small dose of mirapex. It is a parkinson drug. Cannabis is a life saver.
My pain increases with cold weather. I also can have shooting sciatic down my right leg which burns aches and last all night long. Wake up to find it's gone on the right leg, but now I have pain in my left leg. I'm having a vidio conference with a neurologist, to talk about my pain, numbness and tingling in my hands.
