I've finally got an appointment after 2 years and am so excited to see what this doctor can see and recommend.

That's wonderful! I hope it goes well and that it is the start to your feeling better and better. While I haven't had this specific kind of appointment, I think I would want to have written down:
the length of time I've had pain
the types of pain I experience
what I have tried over the years
what has helped and what hasn't (including medications and physical therapy for example)
what intolerances and allergies I have

Only a suggestion is to write down a list of all medications taken, surgeries,….. then give to doctor.

They will go over everything you are already taking and ask about medication intolerance or allergies.I if your not happy asked to he transferred to a different pain management specialist. I refused to appointments with the pain management center in my area because the only three medications they were offering me were very unsafe for me. I was offered oxycodone , moraphine, and and medical marijuana. Two causes a severe migraine which is are reversed by Benadryl and the third sends me to the emergency room just from it being on someone's clothing
