Nortriptyline, aka Pamelor is really weird. I started on 50 Mgs/night which didn't really work as a sleep medication. I asked for a higher doses, 75mgs, and it didn't work at least 2 nights a week on average, caused me to throw it up, irritated my acid reflux from all the chemicals I take and made my nights nightmarish. I asked for a new sleep med called Bellsomra, tiny gerbil pellets that worked not at all. I couldn't go back to Ambien because it has been associated with Alzheimer's and dementia which is rife in my family and my doctor wasn't comfortable prescribing it for me. My doctor found out that my violent rejection was to the formulation of the 75Mg pill which is different from the 50mg pill. Why they changed the formula between the different doses is beyond me! Out of desperation I tried taking 2 50mg pills each night and it worked a splendidly. I fall asleep within 1/2 hour of taking it, I almost always sleep through the night and my mornings are improving very slowly. For about the last 1 1/2 year I've had to double the dose of hydromorphone to get out of bed in the AM but I've been able to reduce my intake of the opioid during the rest of the day. I added a dose of about 15mgs of CBD oil, a dropper full, at night in the hopes of improving my dreadful mornings and I'm happy to report that this past week, despite a bad flareup from trigger point shots, I've been able to return to the single dose of the opioid to climb out of bed and are now using lidocaine and gentle stretches to undo the pain in my lower back. The prescription of Nortripyline I take is now 100mgs/night although the maximum recommended dose is 75mgs. This isn't unusual for me because when I began treatment in '93 I needed a double dose of Ambien because I am incapable of any sleep without medication. It doesn't happen at all. Sleep, I was told is the single most important component of pain mitigation. If Ambien is safe for you to take do so but you might otherwise consider the higher dose of Nortriptyline. Rest well tonight.
